Sentara Quality Care Network partners with Humana to offer value-based health care in Charlottesville

The Sentara Quality Care Network (SQCN) has teamed with national health and well-being company Humana Inc. (NYSE: HUM) to offer a new value-based healthcare model to Medicare Advantage members in Charlottesville. The new approach is part of a fast-growing national trend to enable physicians and specialists to better manage their patients’ care.

Through the new partnership, Humana will work closely with independent and hospital employed healthcare providers in Charlottesville who are affiliated with the SQCN clinically integrated network. SQCN and Humana will provide doctors with powerful population health data and analysis and Humana members with disease management programs, health coaching and wellness programs to help them achieve their best health. Value-based healthcare is part of the industry’s movement toward a healthcare delivery model that focuses on quality, long term well-being and a more coordinated healthcare experience.

Humana currently has about 10,000 Medicare Advantage members in the Charlottesville area and is expected to see continued growth in 2016, according to Humana’s Virginia Medicare President Neil Steffens. SQCN, which began in 2013, currently manages 35,000 (increasing to 75,000 in 2016) individuals across Virginia and is comprised of more than 2,500 healthcare providers. The goal of the network is to improve clinical quality outcomes specific to the needs of a population, such as Medicare Advantage members, and to provide the right care at the right time by the right provider through a physician led commitment to focused care management and the coordination of care. In this model, patient outcomes are continuously measured and evidence-based methods for improving outcomes are applied while efficiencies for both patients and providers are gained.

The agreement includes value-based payment incentives tied to performance improvement and quality outcomes, emphasizing standard measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS), including breast cancer screening, colorectal screening, comprehensive diabetes care and high-risk medications.
