Thank you for being a member of SQCN, a clinician led Clinically Integrated Network (CIN) that encourages colleagues to collaborate to improve the quality, efficiency, and access of care. The SQCN network offers care management services like registered nurses, pharmacists, health coaches, social workers, and diabetes educators to meet the clinical and financial metrics of the network. These services can also boost practice capabilities and staffing, when needed.
